The World Health Organization has confirmed that Southeast Asia has become the first region to completely eliminate trachoma as a public health threat, marking a historic milestone in global disease eradication efforts. The achievement became official with Timor-Leste's recent certification, making it the final nation in the WHO-defined Southeast Asian region to reach elimination status.

More than 2 billion people across the region now face minimal risk of contracting the bacterial infection, with healthcare systems in place to swiftly identify and address any future cases. Trachoma, caused by the bacteria Chlamydia trachomatis, represents the primary infectious cause of blindness worldwide.

"The elimination of trachoma from the South-East Asia Region is a significant step towards our global target of eliminating trachoma worldwide by 2030," Dr. Tedros Adhanom Ghebreyesus, WHO Director-General, said in a statement. "I congratulate Timor-Leste on this important achievement."

The disease operates through a devastating mechanism: repeated infections create scarring on the inner eyelid, eventually causing eyelashes to turn inward in a condition called trichiasis. This inward turning ultimately leads to blindness if left untreated. Communities lacking adequate water, sanitation, hygiene products, and healthcare access face the highest risk, making trachoma a disease closely linked to poverty and resource inequality.

Timor-Leste's path to certification required years of rigorous documentation and surveillance. Between 2015 and 2017, health officials conducted comprehensive investigations examining children on Atauro Island, screening patients at eye clinics nationwide, and performing targeted house-to-house searches. These efforts uncovered no evidence of the disease.

The island nation strengthened its case through additional research in subsequent years. A 2025 survey analyzed 4,949 samples from children aged 1 to 5 years across all 13 municipalities. Laboratory testing revealed no transmission occurring at levels that would warrant public health intervention focused on trachoma.

"Timor-Leste has once again demonstrated that strong national leadership, high-quality evidence and integrated health services can deliver lasting public health gains," said Dr. Nilesh Buddha, Officer-in-Charge at WHO Southeast Asia Regional Office, in a statement. "The country's approach also shows the value of integrating surveillance across diseases so that limited resources produce the greatest possible benefit for communities."

The regional elimination represents a decade-long cascade of victories. Laos DPR became the first Southeast Asian nation to achieve elimination status in 2017. Nepal followed in 2018, with Myanmar reaching the milestone in 2020 and India joining in 2024. Vietnam, Cambodia, Bangladesh, Sri Lanka, Maldives, Bhutan, and Thailand had previously achieved elimination as well.

The WHO validation process demands extensive evidence demonstrating both the absence of trachoma as a public health problem and the capacity of national medical systems to identify, investigate, and manage any cases that might emerge in the future. This multi-year verification ensures that elimination status reflects sustainable healthcare infrastructure rather than temporary success.

Southeast Asia's achievement provides momentum for the global campaign to eliminate trachoma by 2030, demonstrating that coordinated public health efforts can overcome diseases rooted in poverty and inadequate infrastructure. The regional success offers a blueprint for other areas still combating the blinding infection.
